It's the one that is mono with a seperate treble circuit, and it has two 12" speakers and one Jensen R103 metal horn tweeter. Sinse you are trying to find an exact replacement, well good luck! I'm not saying that its impossible, but sinse you are looking for a 15 inch for a seperate baffle project, almost any Magnavox 15 inch speaker will most likely work great for you.

This must have been an upper model, given the 6 knobs and extra row of buttons not seen on lesser models.
